肝硬変患者における体液貯留，腎機能障害のマネジメント

Point
◎肝硬変患者における体液管理は，塩分制限と利尿薬加療が主軸となる．
◎スピロノラクトン，フロセミド内服に対し反応不良の場合，トルバプタンの内服を検討する．
◎内服加療で反応不良の場合はスピロノラクトン，フロセミド静注を検討する．
◎体液貯留が問題となる肝硬変患者では，利尿薬での加療や肝硬変そのものによる腎機能障害の管理も重要となる．
◎末期肝硬変患者における腎障害では，利尿薬の中止とアルブミンへの反応の有無が肝腎症候群の鑑別に重要である．
