眼窩内腫瘍への手術アプローチ

Ⅰ．はじめに

　眼窩内の腫瘍性病変に対する手術アプローチは，①前頭開頭後に眼窩の上壁を除去して，眼窩内に進入する経頭蓋到達法，②眼窩側方に皮膚切開を加え眼窩の側壁を除去する側方到達法，③上ないし下眼瞼に皮膚切開をおいた後に眼球を圧排し，病変に到達する前方到達法，に大別される．このうち前方到達法は，眼球周辺の腫瘍に対する手術法となるが，得られる視野は狭くその適応も限られたものとなる．一方，経頭蓋到達法の適応は広く，本法は眼窩内に限局する病変にとどまらず，眼窩内から視神経管・上眼窩裂，さらに，頭蓋内へ伸展する病変をも切除可能とするものである^3-5,6,8,10)．ただし，病変が視神経の外側に限局している場合には，側方到達法は経頭蓋到達法に比べ侵襲が少なく，有用な手術アプローチとなる^1,2,7,9,10)．本稿では，これらの眼窩内病変に対する手術法のうち経頭蓋および側方到達法について，その外科解剖を中心に手術手技を解説する．
