血管温存に留意した神経膠腫摘出術

Ⅰ．はじめに

　神経膠腫の治療を論議するうえで，手術適応とその方法の標準化が行われていないことが大きな問題である．確かに1つとして同じものがないのが神経膠腫であるが，それらの特徴をまとめ，分類し，それぞれに対する最適と考えられる手術術式を確立する必要がある．手術においては常に突発事態に対しての適応能力は求められるが，可能な限り定型的に行われるべきで，注意点を列挙された手術を論理的に行うことで合併症は少なくなる．

　1990年代の半ば以降，脳機能マッピング，ニューロナビゲーションシステムといった手術理論が神経膠腫に導入されることにより，それまで手術方法を討論されることが少なかったこの疾患に，手術をどう論理的に行ったらよいのかという議論が導入されたことは大いなる進歩であった．しかし，機能温存を目的とした新しい技術導入が進む一方で，実際にどう神経膠腫を摘出するのか，血管をどう処理して危険を排除して効率よく神経膠腫を摘出するか，という基本操作に対しては十分な検討がなされていないと思う．

　今回は血管処理方法に重点を置いた神経膠腫摘出方法に関しての私見を述べたい．
