側頭骨微小解剖―遺体解剖標本を用いて理解する側頭骨解剖

Ⅰ．はじめに

　経錐体骨法は頭蓋底部腫瘍，後頭蓋窩の血管性病変に対して用いられる機会が増えつつあり，一般的な手技として定着しつつある^9,17,19)．その進入部位，削除範囲によって様々なバリエーションがあるが，大別して中頭蓋窩経由で錐体骨を削除して到達する方法anterior transpetrosal approach ^2,8,10)と，錐体骨側面を削除し後頭蓋窩よりに経由する方法posterior transpetrosal approach ^5,12,13)とがある．種々のアプローチにおける手術法についてはそれぞれの文献を参照されたい．いずれのアプローチにおいても，側頭骨内の構造物を損傷せずに，あるいは意図的に削除して適切な術野を得ることが重要である．近年のナビゲーションシステムの進歩により，側頭骨内での構造物の確認は容易になった．しかし側頭骨の外科解剖を理解することが重要であることには変わりはなく，むしろ支援装置の発達した現在においても術者は支援装置に頼りすぎることなく外科解剖の把握に努めるべきである．複雑な側頭骨解剖においては，その中心に位置する顔面神経の走行と内耳諸器官を中心に考えると理解しやすい．その際，本来耳鼻科の領域でもある側頭骨解剖においては普段脳神経外科医にとって馴染みの少ない耳科領域の解剖も関与してくる．昨今，外科解剖を理解するために各地で遺体解剖実習が開催されており，側頭骨解剖を理解するために役に立っている．本稿では顔面神経の走行と内耳諸器官，周囲構造物との関係を中心に，遺体解剖標本作成過程を通じて，側頭骨解剖について解説する．
