Pterional approachのためのcraniotomy

Ⅰ．はじめに

　Pterional approachはYasargilらによって報告²²⁾されて以来，脳神経外科の臨床において最も頻繁に行われている基本的approachであり，orbitozygomatic approach¹⁴⁾，temporo-polar approach^16,17)，extradural temporopolar approach⁵⁾などにも応用されている．当教室では，術後の合併症予防と美容的な配慮を熟慮しつつ，いかに顕微鏡手術に必要十分な術野を得るかを目標にいくらかの工夫をして本approachを用いている．そして，特に主観的な深さを軽減するためにすり鉢状の術野を得ることを重要視しており，顕微鏡下手術では160mmの短いbayonet micro scissorsを好んで用いている（Fig. 1）．本稿では，われわれのpterional approachのためのcraniotomyの方法とその工夫を文献的考察を加えてステップごとに概説する．
