硬膜動静脈瘻の治療戦略

硬膜動静脈瘻(dural arteriovenous fistula,DAVF)は,外傷,静脈洞閉塞,静脈圧亢進との関連が示唆され,現在では後天性疾患と考えられている6,25).硬膜動静脈奇形(dural arteriovenous malformation)と呼ばれることもあるが,後天性疾患と考えると正しい名称ではない.一方,稀な疾患ではあるが静脈洞の発生異常に伴う動静脈シャント疾患が新生児に認められるが,これらは真の意味での硬膜動静脈奇形でありDAVFとは区別すべきである4).DAVFの治療は血管内治療が中心となるが,病変部へのアプローチ法は多岐にわたり,また外科的治療,放射線治療も有効である.本章ではDAVFに対する治療戦略を解説する.
