特発性脳脊髄液減少症

Ⅰ．は じ め に

　特発性脳脊髄液減少症（spontaneous cerebrospinal fluid hypovolemia：spontaneous CSF hypovolemia）は髄液圧低下による頭痛，嘔気，頚部痛などの症状を呈する疾患で起立性頭痛が典型的な症状であり，1938年にSchaltenbrandにより始めて報告されている³⁷⁾．長らくその原因として一定の見解は得られてこなかったが，近年，RI脳槽造影（RI cisternography），CT脊髄造影（CT myelography），MR脊髄撮影（MR myelography）などの画像診断により髄液の漏出が証明される報告が相次ぎ，spontaneous CSF hypovolemiaの発生機序として，硬膜からの髄液漏出が有力視されてきている^{5,7-10,14,16,20,23,27,28,30,35,39,41,43,46)}．そこでspontaneous CSF hypovolemiaについて成因，臨床症状，診断，治療および現在の問題点につき，自験例を加え報告する．
