当院における無剃毛頭蓋手術の実践

Ⅰ．は じ め に

　脳の外科手術において，以前は感染を危惧し専ら全剃毛手術が行われていたが，美容面や社会復帰等への配慮から次第に部分剃毛手術が行われるようになった．しかしながら特に女性の場合，部分剃毛に対してもなお精神的苦痛が少なくはない．近年，術野の剃毛は術後感染率を低下させないというデータに基づき，様々な感染対策のガイドラインにおいて術前の剃毛は不必要，ともすれば感染を助長すると記されている^8,11)．無剃毛手術は1990年代より報告され始め，これまで多数の報告がなされているものの^1,7)，現在においても一部の施設でしか実践されておらず，まだまだ浸透が遅れているのというのが現状である．普及しない理由としては感染への危惧に加え，実際にはどのような術前術後の準備，管理が必要なのか一定した基準が存在しないということが考えられる．当院ではこれまで部分剃毛手術を行ってきたが，2004年10月より完全無剃毛手術を取り入れ，現在ではほぼすべての術式を無剃毛手術で行っている．本稿にてその方法と結果を紹介する．
