脳腫瘍放射線治療後，比較的若年者に晩発性に生じたラクナ梗塞―2症例の報告

Ⅰ．はじめに

　放射線治療後の晩発性の合併症として脳血管障害が生じることはよく知られており，症例報告も散見される．特に脳腫瘍に対して放射線治療を行った場合，腫瘍そのものは長期間に渡りコントロールされているにも関わらず，晩発性脳血管障害により後遺症を生じたり，時には死に至ることもある．今回われわれは，脳腫瘍に対する放射線治療後，晩発性に発生したラクナ梗塞の若年成人2例を経験した．1例は胚腫（germinoma）に対する放射線治療および化学療法を行った17年後に左内包後脚にラクナ梗塞を生じた症例で，もう1例は鞍上部の海綿状血管腫（cavernoma）が疑われる腫瘍に放射線治療を行い15年後に右放線冠にラクナ梗塞を生じた症例である．いずれも診断後，保存的に治療を行い自宅へ復帰できた．いずれの症例も高脂血症はあったものの，高血圧や喫煙の習慣はなく若年時に放射線治療を受けていたことが共通しており，原因として放射線治療を候補に考えた．比較的若年時に脳血管障害を生じたことは，放射線治療が誘因として重要であることを示唆するものと考えるので，これら2症例について，放射線治療後ラクナ梗塞の発症機序，放射線治療後晩発性障害としての脳血管障害を中心に文献的考察を含め検討を加えたので報告する．
