多発性嚢胞腎患者における頭蓋外内頸動脈閉塞に合併した同側頭蓋内嚢状内頸動脈瘤の1例

Ⅰ．はじめに

　内頸動脈狭窄もしくは閉塞患者において脳動脈瘤を合併する頻度は健常人より高く，またその動脈瘤発生部位は閉塞動脈とは別の動脈領域に多く発生する^{2,4-6,8-10,12,13,15-17)}．これは動脈閉塞による脳還流の血行動態が変化することで，それまでとは異なるhemodynamic stressが血管壁に加わることにより動脈瘤が形成されるためであると推察されている．今回，筆者らは多発性嚢胞腎（polycystic kidney disease：PKD）による慢性腎不全患者において，頭蓋外内頸動脈閉塞に，同側内頸動脈の頭蓋内嚢状内頸動脈瘤を伴った症例を経験した．これまでの一側内頸動脈閉塞に合併する動脈瘤に比し特殊であると考えられたため，脳動脈瘤の発生機序に関連して若干の文献的考察を加えて報告する．
