頭頂葉腫瘍性病変の外科治療

Ⅰ．序論

　悪性神経膠腫の基本的な治療指針は可及的に外科的切除を行い，放射線化学療法の後療法，維持療法を追加することである．一方で，個々の神経，高次脳機能をなるべく温存する合併症の少ない治療が求められており，近年，覚醒下手術による機能マッピングの有効性が報告されている^4,28)．これまでに運動，視覚，言語機能マッピング^17,18)を併用した外科手術は数多く報告されているが，感覚障害，失行，失認などが起こりうる頭頂葉腫瘍の外科治療²²⁾，機能マッピング^{5,10,12,16,20,21)}についての報告は少ない．頭頂葉のてんかん外科，腫瘍外科手術後に，皮質性の感覚運動障害や視野障害のみならず，失語や失認などが生じたという報告があること^13,24,25)から，筆者らの施設では，言語機能に加えて頭頂葉機能の温存を目的に，覚醒下あるいは慢性電極留置による頭頂葉機能マッピングを追加し，その結果を皮質切開部位や摘出範囲に反映させている．今回筆者らは，頭頂葉腫瘍性病変に対する外科治療に必要な機能解剖に加え，頭頂葉機能マッピングを施行した自験例の知見について記述する．
