極めて不安定な状態におかれた腰仙椎移行部病変に対するintrasacral fixation（Jackson法）の有用性

Ⅰ．はじめに

　腰仙椎間は，仙骨頭側面が腹側に傾斜し重力線に対して大きな剪断力が作用するため固定しがたく，また骨癒合率も他の高位に比べて低いとされる^11,14,15)．この不安定要因に拮抗すべくこれまでにも種々の固定法が開発されてきた．Jacksonによるintrasacral fixation⁸⁾（Fig. 1）では，岬角を抜くように刺入した仙骨スクリューと仙骨スクリュー頭部より仙骨外側部に刺入したロッドが仙骨を強力に保持し，ロッド先端部が仙腸関節部で押さえられる，いわゆるiliac buttress効果⁹⁾により，腰仙部での屈曲負荷に対する固定性を維持できるとされる．われわれは腰仙椎が極めて不安定な状態を強いられた3症例に対して本法を応用し，その有用性を認めたので報告する．
