日本と米国の脳神経外科診療の違い―第1回

Ⅰ．はじめに

　「医学」は科学であるが，「医療」は社会文化の一部であり¹²⁾，その国の政府の医療政策に依存している．また，医療業界は医療政策，医療制度によってコントロールされている．米国の脳神経外科医は官僚主義の医療政策，患者からの訴訟，民間保険会社と公的保険機関からの診療報酬支払い制限^3,4,5)という厳しい環境の中で診療に当たっていることは本誌で述べた¹³⁾．このように世界各国で独自の社会文化を背景とした「医療」が行われているが，部分的に共通するものもある．

　米国の医療制度，診療の実態を知ることはわが国の医療の将来を考える際の一助となる．米国の制度を参考に日本に導入された最近の例として，診療報酬包括支払い制度（DPC）と臨床研修医のマッチング制度が挙げられる．筆者は第1回（サンディエゴ，2003）と第2回（名古屋，2004）日米脳神経外科友好シンポジウム，そして第64回日本脳神経外科学会（横浜，2005）において日米の医療保険制度について講演の機会を与えられた^14,15)．その際の検討，議論と米国脳神経外科医から得た情報をもとに，今回日米の比較を行った．第1回では，米国脳神経外科の診療の実態について，脳神経外科医の適数，新規採用，診療形態，脳神経外科専門病院，施行されている手術を中心に述べる．
