松果体病変に対するinfratentorial supracerebellar approach

Ⅰ．はじめに

　松果体および松果体近傍病変に対するapproachとしては，①infratentorial supracerebellar approach, ②occipital transtentorial approach, ③posterior transcallosal approach, ④posterior transcortical approachなどが挙げられ，それぞれのapproachに長所と短所，到達限界があるが^2,13)，主に用いられるのはinfratentorial supracerebellar approachとoccipital transtentorial approachである．各approachの比較は他書に譲り^2,13)，本稿では，現在われわれが松果体部病変に対して行っている坐位によるinfratentorial supracerebellar approachの特徴と，手術に必要な局所解剖，麻酔法および空気塞栓に対する対策，手術手技について解説する．
