Ⅰ．はじめに

　前交通動脈瘤は脳動脈瘤全体の約30％を占め，脳動脈瘤の中でも手術機会が最も多いものの1つである．したがってその手術法はほぼ確立した観があるにもかかわらず，アプローチ法の選択，pterional approach（以下PAと略す）における開頭側の決定，半球間裂到達法（interhemishpheric approach）（以下IHAと略す）における進入角度など，術者の経験と好みに依存する不確定要素も少なくはない．こうしたことに加え，本動脈瘤は前方循環動脈瘤のうちで最も深く，かつ関与する動脈が最も多いことなどから，これからクリッピングを習得しようとする脳神経外科医にとっては困難かつ難解なものの1つである．

　こうしたことを前提に，さらに本稿の読者層はクリッピングに関する比較的初級者が主体であると想定し，スタンダードな手術法とその背景となる基本的な考え方，および理解のために必要な解剖学を中心に解説を加える．さらに前述の不確定要素に関してはいずれを選択するかについて私見および文献的見解をもとに現在における標準的な考え方を示した．
