Ⅰ．はじめに

　頭蓋底は脳側から見た内頭蓋底と顔面側からみた外頭蓋底の両側面を持つ．内頭蓋底に比較し，外頭蓋底は顔面頭蓋に向けて複雑で入り組んだ形態をしている．この外頭蓋底につづいて顔面頭蓋の前部には鼻副鼻腔，眼窩，上咽頭，口腔などが存在し，後部には側頭下窩，翼口蓋窩，副咽頭間隙などの領域や斜台につづく上位頸椎などが存在する⁶⁾．頭蓋底内外に跨がる病変に対して，岸本は頭蓋底・顔面深部外科として報告・記載している⁶⁾．近年，これらに対して，耳鼻咽喉科頭頸部外科（以下頭頸部外科と略す），脳神経外科，形成外科など複数の診療科が協力して手術を行うinterdisciplinary approachの重要性が認識されている．頭頸部外科，ならびに，脳神経外科にとって頭蓋底内外に跨がる病変は境界領域であり，また，形成外科は整容面を考慮した術野の展開だけでなく，手術に伴う頭蓋内と頭蓋外との交通を遮断するために重要な役割を果たす．本稿では，合同手術のうち，側頭骨手術を除く，前頭蓋底，中頭蓋底手術について，脳神経外科の立場からみた手術手技について概説する．
