Ⅰ．はじめに

　ヒトの松果体は，重量100mg前後の小さな器官にすぎないが，この部分は脳腫瘍の好発部位であり¹³⁾，特異な臨床症状ともあいまって脳神経外科の臨床においては重要な臓器である．松果体は，大脳の最も深い位置に存在すること，間脳や脳幹に近いこと，また重要な脳動静脈に囲まれていることから，以前は不可侵の領域とされてきた．

　後頭経小脳天幕アプローチ（occipital transtentorial approach：OTA）は，後頭開頭により後頭葉内側面と大脳鎌の間を通り，一側の小脳天幕を切開して松果体部に到るアプローチ方法である⁷⁾．われわれは1995年以降，松果体部腫瘍の大部分をこのOTAを用いて手術してきた．本稿では松果体部腫瘍に対するOTAの選択基準，手術法の実際，本法におけるピットフォールとその克服，自験の成績をまとめる．
