半球離断術までの歴史と現況

Ⅰ．はじめに

　半球離断術（hemispherotomy）が成立するまでの歴史的経過を理解する上で重要なことは，各種の術式を正しく理解することである．一側半球に広範囲に焦点が分布している場合に適用される外科的手段として，解剖学的半球切除術（anatomical hemispherectomy），半球皮質切除術（hemidecortication or hemicorticectomy），機能的半球切除術（functional hemispherectomy），半球離断術（hemispherotomy），などの各種の方法が開発されてきた．

　この中で，物理的に基底核を残して半球組織すべてを切除してしまう方法が解剖学的半球切除術である（Fig. 1）．また，白質は残して，灰白質のみを皮をむくように切除する方法が半球皮質切除術（Fig. 2）である²⁾．この術式はhemicorticectomyと呼称される場合もあり，Winstonらは“de-gloving”dissection around the lateral ventricle という表現が，この術式の概念を最もよく示していると記載している³³⁾．いわゆる古典的半球切除術は，この解剖学的半球切除術と半球皮質切除術の両者のいずれかを意味していることが多い．

　機能的半球切除術は，Rasmussenが解剖学的半球切除術の後遺症を予防するために考案した手術法で，前頭極と後頭極のみを残して，中間の部分は全部切除してしまう手技である（Fig. 3）²³⁾．かなり切除範囲の大きい手術で，半球離断術とは基本的に異なっている．機能的という名称がついていることから，機能的半球切除術と半球離断術が同義に用いられている場合を散見するが，両者はまったく異なる手術手技であり，厳密に区別する必要がある．

　半球離断術は，従来の手術手技が広範囲の脳組織の切除を主体にしていたのに対して，最小限の脳組織の切除腔から半球の連絡線維を遮断する方法で，大きな切除腔が形成されないこと，また手術侵襲が小さいことなどから，小児にも比較的安全に適用できる画期的手術法である．この半球離断術の概念は，1992年にDelalandeらにより初めて提唱されたものである⁵⁾．

　半球離断術は，アプローチの方向からvertical approachとlateral approachに分類されるが，神経線維の離断を主体とする半球離断術の根本的理念には相違がない．これらの手術法については，後で詳細に触れることにする．

　以上のような経緯から，半球を完全に遮断する手術法として，

　①解剖学的半球切除術anatomical hemispherectomy

　②半球皮質切除術hemidecortication or hemicorticectomy

　③機能的半球切除術functional hemispherectomy

　④半球離断術hemispherotomy（vertical approach/lateral approach）

の4種類があることをしっかり念頭に入れておく必要がある¹⁶⁾．そして，これらの術式は，未だに現在においても，すべて用いられており，時代の変遷とともに術式が進化して，理想的術式として半球離断術に到達したと考えるのは，必ずしも現状の正しい理解とは言いかねる．
