島限を温存した経シルビウス裂到達法の選択的海馬扁桃体摘出術―手術手技と海馬硬化症70例に対する術後の高次脳機能について

Ⅰ.はじめに

 側頭葉てんかんは外科治療による発作抑制率が高く,その手術法には最も標準的に行われている前側頭葉切除による海馬扁桃体摘出術(anterior temporal lobectomy:ATL)と側頭葉内側構造のみを摘出する選択的海馬扁桃体摘出術(selective amygdalohippocampectomy:SA)がある.ATLは,特に言語優位側の手術では,術後の言語性記銘力の低下や側頭葉内の視放線の一部損傷による自覚症状は認めないが,対側の上1/4盲の出現が問題となる.これらのATLの合併症を回避するためにSAが開発されたが,海馬扁桃体を中心とする側頭葉内側部への到達経路により,種々の手術法が報告されている.SAの最初の報告は1958年のNiemeyer 19)の中側頭回経由の経皮質到達法である.その後,Wiser とYasargil 28)が経シルビウス裂到達法による選択的海馬扁桃体摘出術(transsylvian SA:TSA)を発表した.しかし,彼らのTSAにおいては側頭幹を切断することと島限の直下を走行するuncinate fasciculusを離断することにより,記銘力温存に不利益をもたらしていると報告されている8)

 当施設では2001年より側頭葉てんかんに対して,島限を温存した経シルビウス裂到達法による選択的海馬扁桃体摘出術(TSA preserving the limen insula:TSA-PLI)を行い,発作予後および術後の高次脳機能について良好な結果を得ている.そこで本稿では代表例の術中写真を提示しながら,本手術法の要点と海馬硬化症70例の発作予後および術前後の高次脳機能を含む手術治療成績について述べる.
