Ⅰ．はじめに

　今日，広頚脳動脈瘤の塞栓術において行われるneck remodelingでは，バルーンの使用やステントの併用を中心としたさまざまな工夫がなされている^1,2,7)．Catheter-assisted techniqueもその1つであるが，その方法についての詳細な記述などのまとまった報告は少ないのが現状である³⁾．今回，椎骨動脈─後下小脳動脈（posterior inferior cerebellar artery：PICA）分岐部に発生した破裂広頚動脈瘤の塞栓術に際して，catheter-assisted techniqueにより瘤内塞栓を行った症例を提示し，われわれの行っている方法を紹介する．
