Suction decompression法を併用した巨大・大型paraclinoid動脈瘤のクリッピング術―そのセッティングとクリッピング時の留意点

Ⅰ．はじめに

　Paraclinoid動脈瘤は，一般に硬膜輪（distal dural ring）から内頚動脈後交通動脈分岐部までにneckが存在する動脈瘤を指すが⁶⁾，大型（15mm以上）あるいは巨大（25mm以上）になる頻度が他の部位の動脈瘤より高いことから，治療困難な動脈瘤の1つと考えられている^24,25)．大型・巨大動脈瘤では，動脈瘤の圧を十分に減じるために，近位部の内頚動脈から血液を逆行性に吸引し，その間にクリッピングを行うsuction decompression法^{2,5,8,10,17,25)}を併用することが重要な手術手技である．これにより，内頚動脈を形成しながらのクリッピングが可能となり，眼動脈の血流を温存し，視神経へのダメージも少なくすることができる．

　筆者らは，これまで本法を併用しての巨大・大型paraclinoid動脈瘤のクリッピングを24症例に行い良好な成績をあげてきたので，本稿では，そのセッティングとクリッピング時の留意点を中心に解説する．
