Ⅰ．はじめに

　近年，画像診断技術の進歩や脳ドックの普及などにより，無症候性の脳梗塞が診断される機会が増えている．また，ライフスタイルの欧米化や危険因子の変化に伴うアテローム硬化症の増加などもあり，TIA（transient ischemic attack）症例に遭遇する機会が多い．また，TIAについては，最近の画像診断の進歩に伴い，その概念も変化しつつある．

　そこで本稿では，無症候性ないし軽症脳梗塞とTIA，おのおのの病態について最近の知見をふまえ，概念や診療方針などについて述べる．
