4．めまいを唯一の症状とする末梢性と中枢性疾患

I．めまい疾患における最近の動向

　めまいを主訴に救急外来や脳神経外科，神経内科，耳鼻咽喉科を受診する患者は少なくない．高齢化に伴って高齢者のめまいも増えている．特に，めまいが急に発症し，しかも，その人にとってはじめてのことであれば，回転性めまい（vertigo）であれ，浮動性めまい（dizziness）であれ，救急外来受診となる可能性は高い．これらの中には，めまい以外に症状がなく，診断に難渋する例も少なくない．このような例の背後に隠れている，生命にとって危険な緊急に対応を要する疾患を効率的に診断することは，救急医やめまいを扱う診療科の医師の役割でもある．救急外来ではとりあえずCT検査で脳出血が否定されれば，後日，耳鼻科，神経内科の受診を勧めるのが一般的である．しかし，めまいのみを主訴とする末梢性めまい疾患や神経学的所見のない脳硬塞があること，後者では少し時間差をもって後日神経症候が生じ神経学的所見がとらえられる例のあることも知っておくことが必要である．

　本誌は主に脳神経外科医を対象としていると考えるので，めまい症例でも明らかな神経学的異常を伴うものは除外し，めまいをほとんど単独に訴える疾患（isolated vertigo or dizziness）の最近の動向について述べてみたい．
