舌下神経シュワン細胞腫の手術戦略

Ⅰ．はじめに

　舌下神経から発生するシュワン細胞腫は稀ではあるが，その進展様式から手術治療を考えたときに，非常に興味深い．本腫瘍は，頭蓋内，舌下神経管内，頭蓋外に進展し得る．舌下神経管は，後頭顆を横切っているという解剖学的特徴がある．また，舌下神経に限ったことではないが，脳神経は，頭蓋内では，epineuriumに覆われておらず，舌下神経管に入るところで，硬膜，くも膜に連なり，epineurium，perineuriumに覆われるようになる．これらの解剖学的特長は，舌下神経シュワン細胞腫の手術戦略を考える上で，極めて重要である．

　前述のように，舌下神経管は後頭顆を横切っている．腫瘍が舌下神経管内で発育すると，舌下神経管が拡張する．これはつまり，後頭顆自体を破壊することを意味する．これまでに，本腫瘍に対する手術戦略がいくつか報告されているが，後頭骨環椎関節の安定性に関しては，ほとんど議論されていなかった^4,7-10)．われわれは最近，後頭骨環椎関節の安定性を考慮した，舌下神経シュワン細胞腫の手術戦略を報告した⁵⁾．本稿では，舌下神経シュワン細胞腫の治療方針，特に舌下神経管内の腫瘍に対する手術戦略について，condylar fossaを経由する他のアプローチと比較して解説する．
