Occipital transtentorial approachによって松果体部腫瘍摘出後に静脈洞交会部血栓症による視野障害を起こした1例

Ⅰ．はじめに

　松果体部腫瘍に対する手術アプローチとしてoccipital transtentorial approach（OTA）によって腫瘍摘出を行うことは今や一般的な方法として行われている^3,7,10)．OTAによる手術合併症として視野障害，複視・眼球運動障害が報告されてきたが，原因としては術中の後頭葉障害や脳幹部障害によるとされている¹⁰⁾．

　今回われわれは，松果体部腫瘍に対してOTAによる腫瘍摘出を行ったところ，術後に一過性の同名半盲を起こした症例を経験した．手術による直接の後頭葉障害ではなく，上矢状静脈洞から静脈洞交会部の血栓症が原因に関与していると考えられたため報告する．
