出血発症の左A1解離性動脈瘤に対しproximal clippingにA3-A3バイパス術を併用した1例

Ⅰ．はじめに

　動脈解離（解離性動脈瘤）は脳卒中の原因として重要であるが，診断や治療が困難な症例が存在する．本邦では，動脈解離は内頚動脈（ICA：internal carotid artery）系より椎骨脳底動脈系に多く，また頭蓋内に多いとされている⁹⁾．動脈解離の原因として，先天的要因による動脈壁の異常，動脈の走行異常，軽微な外傷，感染などが考えられている⁵⁾．

　今回，われわれは出血発症の左A1解離性動脈瘤を経験した．その治療としてproximal clippingにA3-A3バイパス術を併用し，良好な経過を得た．前大脳動脈（ACA：anterior cerebral artery）の動脈解離の報告は少なく，未だその治療法は確立していない¹¹⁾．そこで文献的考察を加え，本症例を報告する．
