Medullomyoblastomaの1例

Ⅰ．はじめに

　Medullomyoblastomaは小児の小脳正中領域に発生する極めて稀な腫瘍であり，古典的なmedulloblastoma様の小型類円形細胞の増殖に混じって横紋筋線維，稀には平滑筋線維への分化を認めるという特異な組織パターンを示す．2007年の中枢神経系腫瘍のWHO分類ではmedulloblastomaの中に複数個の腫瘍亜型，組織パターンがあり，medullomyoblastomaは組織パターンの1つでmedulloblastoma with myogenic differentiationと分類されている³⁾．本腫瘍は1933年のMarinescoとGoldsteinによる最初の報告以来¹³⁾，今日まで50例余りの報告をみるにすぎず，本腫瘍の発生起源に関しても奇形腫説，血管周囲の内皮細胞や間葉系細胞由来説，原始神経外胚葉細胞由来説などが考えられているが，今なお明らかではない．

　今回われわれはmedullomyoblastomaの1例を経験したので，文献的考察を加えて報告する．
