頚静脈孔部腫瘍患者に生じた同側顔面痙攣に微小血管減圧術が有用であった1例

Ⅰ．はじめに

　顔面痙攣は間欠的に不随意に生じる片側性の顔面筋の収縮であり，一般的に後下小脳動脈，前下小脳動脈，椎骨動脈を責任血管とした頭蓋内顔面神経根出口帯（root exit zone：REZ）への機械的圧迫により生じる⁶⁾．一方，稀ではあるが小脳橋角部腫瘍，脳動静脈奇形，脳幹神経膠腫に伴う症候性顔面痙攣（symptomatic hemifacial spasm）もこれまで報告されている^14,15)．小脳橋角部腫瘍に伴う症候性顔面痙攣では，腫瘍そのものによる顔面神経への機械的圧迫のほか，腫瘍存在下の血管や，くも膜の偏移がREZに対する機械的圧迫の原因となる場合もあることが報告されており，原疾患である腫瘍摘出により改善が得られたケースも散見される^11,14)．しかし，これまで頚静脈孔部腫瘍患者に顔面痙攣を生じた報告は1例の報告が認められるのみであるため⁸⁾，発症機序，治療法など不明瞭な点も多い．特に治療法に関しては下位脳神経症状が出現していないケースでは，同部腫瘍に対する積極的外科治療の適応は低く，同患者に生じた副次的症状である顔面痙攣に対する治療は悩ましい問題である．今回われわれは，放射線治療後に外来にて経過観察されていた無症候性頚静脈孔部腫瘍患者に同側顔面痙攣を認め，ボツリヌス毒素，内服治療を試みた後に微小血管減圧術を行い，顔面痙攣消失が得られた1例を経験した．症候性顔面痙攣に対する治療に関して考察を加え報告する．
