医原性椎骨動静脈瘻に対して外科治療が奏功した2例

Ⅰ．はじめに

　椎骨動静脈瘻は頭蓋外の椎骨動脈とその周囲の静脈や静脈叢との間に生ずる動静脈瘻である．頻度は比較的稀であり，成因としては，特発性の他に外傷性，動脈硬化，血管炎，放射線などがある^6,12)．このうち最も多いのは医原性を含む外傷性（60％）であり，頚静脈への中心静脈カテーテル挿入時の椎骨動脈誤穿刺はよく知られた原因である^2,9)．治療に関して，近年は血管内治療が優先的に選択される傾向にあり，直達手術の報告例は少ない^2,5)．血管内治療は有効ではあるが，根治性や合併症など問題点も少なからず存在する．今回われわれは低位病変の医原性椎骨動静脈瘻に対して外科治療を行った2症例を呈示し，文献的検討を含めて報告する．
