痙縮に対するITB（intrathecal baclofen）療法でのbaclofen至適投与量の検討

Ⅰ．はじめに

　痙縮は錐体路障害に伴い生じることが多く，筋の伸張速度に依存した伸張反射の亢進状態と解釈され，関節を他動的に速く動かそうとすればするほど伸張された筋の収縮抵抗が増す状態（相動性伸張反射亢進状態）である．その原因疾患は，脊髄損傷，脳性麻痺，頭部外傷，脳血管障害，遺伝性痙性対麻痺（hereditary spastic paraplegia：HSP）など多岐にわたる．さらに痙縮は，重症化すると拘縮や締め付け感や呼吸・睡眠の障害の原因となり，看護やリハビリテーションの妨げとなるので，患者の自立度を高め，介護者の負担軽減のためにも治療する必要がある^8,17,18)．治療としてはbaclofenの経口投与が行われてきたが，眠気などの副作用により十分な投与が困難で，重篤な痙縮には有効ではなかった．1980年代にPennらが，baclofen持続髄注（intrathecal baclofen：ITB）療法の有功性を示した^14,15)．しかしながら，症例によっては痙縮を改善したことで，逆に支持性が悪くなり，歩容が悪化する場合もある．これは薬剤の過量投与が原因とも考えられるが，各疾患におけるITB療法の至適baclofen投与量に関しては不明な点も多い．そこでわれわれは，自験16例で，病態ごとの至適baclofen投与量に関して検討したので報告する．
