Branch atheromatous disease（BAD）の病態と治療

Ⅰ．はじめに

　急性期に穿通枝領域に特徴的な梗塞巣を呈するbranch atheromatous disease（BAD）は治療抵抗性に進行性脳卒中を呈する場合が多く，急性期画像診断の進歩とともに注目されている病態である．しかし，BADはわが国で多くの臨床報告がなされているが，国際的には病型としては未だに確立されていないのが現状である．

　また，わが国ではBADを臨床的に発症直後から進行性の経過をとる脳梗塞として，臨床経過までを含めて定義して使用されることも多く，その概念には混乱がみられる．われわれは，J-BAD Registryとして，BADを放射線学的画像により定義してその臨床的特徴や危険因子を検討した¹¹⁾．その結果を中心にBADについて概説する．
