生体肝移植後に自然消失した脳動静脈奇形の1例

Ⅰ．はじめに

　頭蓋内脳動静脈奇形（arteriovenous malformation：AVM）の頻度は0.02～0.05％とされており，通常は先天的で時間経過とともに徐々に増大することが多いと考えられている^9,20)．頭蓋内AVMの発症機転としてはけいれん発作と巣症状のほか，破裂による出血が最も頻度が高い．出血発症の危険性は局在部位や出血の既往の有無に左右され，年間出血発症率は2～4％でそのうち30％が死の転帰を辿る⁵⁾．根本的な治療法としては，そのサイズ，局在，導出静脈パターンなどを勘案して外科的手術・放射線治療・血管内治療などが実施されているが，いずれの方法においても合併症の危険性があることは周知の事実である⁹⁾．

　頭蓋内AVMの自然消失は比較的稀で，頭蓋内AVM全体の0.8～3％と報告されている⁹⁾．そのうちの約70％が，症候性の脳内出血やくも膜下出血の後に消失したと報告されている¹⁾．今回，われわれは重篤な肝硬変に合併して発見された中脳近傍のAVMが生体肝移植を契機に消失した症例を経験したので，そのメカニズムの考察を含めて報告する．
