Ⅰ．はじめに

　脳血管内治療は低侵襲治療としてその適応を拡大しつつある．しかし治療が無事終了しても，術後に思わぬ合併症で悪化することもあり，周術期管理は重要である．脳血管内治療に共通する特有の問題として，穿刺部合併症，抗血栓療法の併用，造影剤の使用がある．また未破裂脳動脈瘤患者には合併する疾患はほとんどないが，頚動脈ステント患者は，冠動脈疾患をはじめさまざまな動脈硬化性疾患を合併していることが多く，注意が必要である．本稿では，脳血管内治療の周術期管理において重要と思われる事項，および治療頻度が高い頚動脈ステントと未破裂脳動脈瘤の周術期管理の実際について概説する．
