認知症診断の最近の進歩―アルツハイマー病のMRIとPETによる画像診断

Ⅰ．はじめに

　アルツハイマー病をはじめとする認知症は，高齢化社会を迎えるわが国の健康上の最大の心配事の1つである．世界中でさまざまな治療法（治療薬）の研究開発が行われているが，今後どんなに優れた治療法が出現しても，病気が進行して脳細胞が大量に死滅してしまってからでは治療効果にも限界があると考えられる．したがって，早期発見・早期治療が重要となってくることは容易に推察できる．

　近年，アルツハイマー病などの認知症を極めて早期に発見できる診断法として，PETやMRIなどの画像バイオマーカーが注目されている．現在，わが国においては画像バイオマーカーを用いたアルツハイマー病の多施設研究であるJapanese Alzheimer's Disease Neuroimaging Initiative （J-ADNI）が進行中である．われわれの施設では，金沢大学脳老化・神経病態学講座との共同研究で，2002年2月より認知症などの脳変性疾患の早期診断法の確立を目指して「石川ブレインイメージング研究」を開始しており，これまで1,265例以上の健常ボランティアと496例以上の認知症（疑い含む）患者のFDG PET，MRIなどを施行してきた．本稿では，これらの経験を踏まえ，MRI/PETなど画像でみる脳老化と認知症について概説する．
