Trappingで治療したsupreme anterior communicating arteryに発生した破裂紡錘状脳動脈瘤の1手術例

Ⅰ．はじめに

　Supreme anterior communicating artery（SAcom）は，脳梁膝部において脳梁周囲動脈（pericallosal artery）と脳梁辺縁動脈（callosomarginal artery）の分岐部で，左右の動脈間を交通する動脈としてLaitinenらにより初めて報告された⁵⁾．その後の報告によると，SAcomは胎生期の遺残や前交通動脈の重複などと考えられており，末梢性前大脳動脈瘤に合併することがあると言われている^3-5,12)．

　SAcomの症例報告自体は非常に稀であり，教科書的にもその存在について記載されているものは極めて少数である⁸⁾．今回，われわれはSAcomに生じた紡錘状破裂脳動脈瘤にtrapping術を施行し，良好な経過を辿った症例を経験したので，現在までの報告に若干の文献的考察を加え報告する．
