巨大延髄背側血管芽腫の1治療例

Ⅰ．はじめに

　血管芽腫（hemangioblastoma）は，2007年のWHO分類では「その他の髄膜関連腫瘍」として記載されており，本邦の統計では全頭蓋内腫瘍の1.7％を占める良性腫瘍である⁴⁾．頭蓋内に発生する大部分の血管芽腫は後頭蓋窩，特に小脳半球，虫部内に発生し，延髄など脳幹部に発生することは稀である^8,19)．頻度的に稀な腫瘍型であることから自然経過が不明なことに加え，同腫瘍型に対する外科治療は，小脳に発生する血管芽腫とは異なり術合併症の発症リスクが高い．頭蓋内血管芽腫のうち20％程度を占めるvon Hippel-Lindau（VHL）病関連血管芽腫を含めて，脳幹部血管芽腫に対する適切な外科治療開始時期に関しては，未だ議論のあるところである．今回われわれは，偶発的に発見された無症候性非VHL延髄背側血管芽腫に対し，経過観察を行い，結果として腫瘍径40mm以上に増大した状態で外科治療を選択しなければならなかった1例を経験した．同腫瘍に対する治療開始時期に関して，文献的考察を踏まえ報告する．
