Ⅰ．はじめに

　今回，合併症のシステマティック・レビューを行うにあたって，試みにデータベースから代表的な脳神経外科手術に伴う合併症を抽出した．予想していたが，日常的に遭遇する合併症に関して，信頼に足る報告はほとんどなかった．例えば，内頚動脈─後交通動脈分岐部の動脈瘤に対するクリッピング術の際にみられる動眼神経麻痺は，それほど稀ではないと想像されるが，2000年以降のすべての論文を渉猟しても，系統的な調査は1編も存在しなかった．あるいは，半球間裂アプローチによる前交通動脈瘤の際の嗅神経障害や感染症に関しても，信頼に足る頻度に言及した研究をみつけることができなかった．

　一般に，合併症に関する論文は少ない．学会発表や学術雑誌などでは，合併症などの負の記録を系統的に記載した報告はそれほど多くなく，成功例などの正の記録が重視される．学問の発展は，成功例という正の記録の点と点を連結して，理論の確立とその検証を行うほうが，実用的で，スピードが速いことを私たちは経験的に知っている．医学は，「健康の増進と疾病の予防，治療」というわかりやすいゴールドスタンダードがあり，成功体験を重視する医学や社会の要請に即時性をもって的確に応えるためには，やむを得なかったという社会的要因もある．p＜0.05のポジティブデータという 「点」 を線で結んできたのが，現代の医学・脳神経外科である．特に，evidence based medicine（EBM）が確立された1960年代以降は，そのスピードに拍車がかかると同時に，一方で，外科的な手技に伴う合併症の研究は，EBMが成り立ちにくいこともあり，むしろ停滞した印象がある．こうした条件の下で，合併症を系統的に検索することは容易ではない．

　しかし，合併症に関する知識は，実際の臨床現場では，正の記録として教科書や論文に記載されてきた事実以上に，極めて実用的で実際的な知識である．また，何より，患者への正しいinformed consentにとって必須の情報を与えてくれる．

　今後，隔月で連載される本シリーズには，このような背景があることを読者の方々にはぜひご理解いただきたい．

　また，重ねて言うまでもないが，本シリーズの利用の最大の目的は，患者への適切なinformed consentを行うデータベースを提供することにある．合併症というものは，頻度の高いものから極めて稀なものまでさまざまであり，個人的な経験だけでは，全貌を把握することはできない．思いがけない合併症も，実は，その多くは既に報告されており，これに関する知識をもつことが，良質な日常診療にとって必須のことである．

　重ねて言うが，本論文は，あくまで，信頼できる論文の総合的な解析に基づいたものであり，個々の症例，個々の施設，個々の術者にとって，適応されるものではない．必要に応じて，referenceとして活用していただきたい．
