（2）未破裂前大脳動脈瘤の外科治療

Ⅰ．はじめに

　合併症のシステマティック・レビュー・シリーズの一環として，前回の内頚動脈瘤に引き続いて，未破裂前大脳動脈瘤の外科治療に伴う合併症に関するシステマティック・レビューを行った．内頚動脈瘤のレビューと同様に予想されたことではあるが，未破裂前大脳動脈瘤に限定した，多施設間にわたる大規模なrandomized controlled trialや，meta-analysisは，存在しなかった．また，予想以上に，合併症に関する記載は，多様かつ報告間でばらつきがあり，合併症の正確な頻度を調査することも困難であった．

　しかしながら，生じ得る合併症を知り，患者に適切なインフォームド・コンセントを実施することは，日常診療において不可欠なことは言うまでもない．また，それ以上に，起こり得る合併症を知ることにより，それを未然に防ぐ努力をすることは，外科治療の水準を高めるうえで重要な意義があると考えられた．さらには，医療の不確実性から考えて，万全な準備と治療を行っても，合併症をゼロにすることは不可能である．しかし，そうした合併症が起こり得ることを前もって認識していることにより，万が一，合併症が起こった場合も，それに対して適切に対処することが可能になる．したがって，稀な合併症も含めて，起こり得る合併症に関する知識は，外科医に求められる必須の知識であるとも考えられた．

　合併症に関する有用な知識は，大規模studyによって明確に数字で示されたmorbidityやmortalityだけではない．これらは，一様に，統一性をもった合併症の定義と強い科学的根拠を有し，その治療成績の全貌と正確な合併症頻度を把握するうえで，極めて重要なものであることは疑いようがない．しかしながら，実際には，個別の動脈瘤を前に，起こり得る合併症について，こうした大規模studyのデータは必ずしも適切な情報を与えてはくれない．個別の動脈瘤に対して，具体的にどのような手術が行われ，どういった合併症が生じたかは，実質的に見えてこない場合も多い．逆に，頻度としては極めて低いものであったとしても，詳細に記述されたsingle case reportが，実質的に有用な情報を提供してくれることもある．

　そのため，本論文の執筆にあたり，多様な論文の中から，生じ得る合併症を可能な限り抽出することに尽力した．抽出された論文も，合併症に注目した主旨のものが多い．そのため，以下に示す合併症率は，一様に高い傾向を示しているかもしれない．その意味では，本論文での合併症率は，あくまでそういった背景のもとで論文の解析を実施して導き出した数値であり，個々の症例，個々の施設，個々の術者にとって，必ずしも適応されるものでもない．必要に応じて，referenceとして活用していただければ幸いである．

　また，本論文は，未破裂前大脳動脈瘤の手術適応の是非，あるいは，pterional approachとinterhemispheric approachのどちらが優れているというような手術法の優劣を比較することを目的としていない．たとえどんなに優れた術者が，どんなに優れた手術法を行ったとしても，合併症は生じ得る．その合併症に関する知識共有が目的であり，その知識をもとに患者に適切な説明がなされることが重要であることを，改めて強調したい．
