術中VEPモニタリングによる視機能の温存―VEPに変化を来した手術手技の検討から

Ⅰ．はじめに

　視覚路に関わる脳神経外科疾患では，視機能の温存や改善が手術の重要な目的である．しかしながら，意に反して術後に視機能が悪化することも経験する．手術中に視機能の正確なモニタリングが施行できれば，視機能障害の回避が可能となり，術後の機能回復にも貢献できる．1970年代から光刺激による視覚誘発電位（visual evoked potential：VEP）の術中モニタリングが試みられてきた⁸⁾が，安定性に乏しく臨床的に有用とはいえなかった⁴⁾．そこで，筆者らは新しい光刺激装置を作製し，網膜電図（electroretinogram：ERG）の同時記録を追加し，propofolを用いた全静脈麻酔を用いたところ，VEPの再現性は良好になり安定したモニタリングが可能となった⁵⁾．本稿では，VEPに変化を来した手術手技を検討することにより，視機能の温存における術中VEPの役割や意義について述べたい．
