高位内頚動脈狭窄症に対する頚動脈内膜剝離術--連続22例の検討

Ⅰ．はじめに

　頚部内頚動脈（internal carotid artery：ICA）狭窄症に対する治療は頚動脈内膜剝離術（carotid endarterectomy：CEA）と頚動脈ステント術（carotid artery stenting：CAS）が存在する．CASはCEAが困難な症例（高位病変，高齢者，CEA後の再狭窄，放射線治療後，全身疾患の合併）で選択されていたが，近年の血管内治療技術の進歩によりそれ以外の症例でもCASが行われるようになってきた．しかし未だCASにおけるdistal embolism¹¹⁾や全周性の石灰化病変¹⁷⁾に対する問題が完全に解決されていない．したがって，当院では高位病変でも積極的にCEAを施行してきた．十分にICA遠位部を露出し，プラークの遠位端を確実に処置することが安全なCEAを施行するにあたり不可欠であるが，高位病変に対しては困難な状況が存在する．しかし，当院では高位病変に対して浅層の構造物を広範囲に丹念に処理することで，経鼻挿管や顎関節脱臼などの特別な方法^1,7)を必要としない通常の手術手技で対応してきた．その手技の成績と術後の有害事象の発生頻度を検討し，報告する．
