化膿性脊椎炎に対して後方固定術を施行した1例

Ⅰ．経験症例

1．症例

　74歳男性，僧帽弁閉鎖不全症に対する僧帽弁置換術（機械弁）の既往がある．1カ月前から持続する背部痛を主訴に他院を受診し，精査目的に入院となった．入院18日後に右下肢麻痺が出現し急速に対麻痺，排尿障害へ進行した．MRIで胸椎硬膜外血腫と診断され緊急転院となった．MRI（Fig.1）では，Th5，6椎体に信号変化を伴う圧潰を認めた．また，Th5，6椎体後面にT1強調像で低信号，T2強調像で高信号を呈する液体貯留を認め，脊髄に軽度の圧迫を伴っていた．以上の所見から化膿性脊椎炎および脊髄硬膜外膿瘍と診断した．
