開頭部位に播種した再発退形成性上衣腫の1例

Ⅰ．はじめに

　Ependymomaは上衣細胞から発生する中枢神経原発腫瘍であり，小児の原発性脳腫瘍の5～10%を占めている^15,16)．The World Health Organization（WHO）では組織学的特徴からependymomaをsubependymoma（WHO grade Ⅰ），myxopapillary ependymoma（WHO grade Ⅰ），ependymoma with cellular, papillary, and clear cell variants（WHO grade Ⅱ），anaplastic ependymoma（WHO grade Ⅲ）の4つのグループに分類している．

　Ependymomaの生命予後は，組織型，発症年齢，手術による摘出率との相関が認められている⁸⁾．したがって，治療は可能な限り腫瘍を摘出することが重要となる¹⁴⁾．この腫瘍に対する化学療法は無効であるが⁶⁾，放射線治療に対する感受性は認められるため，腫瘍摘出後に放射線療法を行うことが一般的である¹⁴⁾．しかし，再発することも多く，そのほとんどは原発巣や脊髄播種でみられるが，中枢神経系以外に転移することは極めて稀である．Ependymomaの中枢神経系以外の転移先は肺や胸膜，肝臓，リンパ節など^3-5,11)であり，手術操作が原因で中枢神経系以外である開頭部位に播種したという報告は，渉猟し得る限りわずか1例であり，極めて珍しい転移形式である²⁾．われわれは手術操作により硬膜外に播種したと考えられたanaplastic ependymomaの1例を経験したので報告する．
