重症頭部外傷治療・管理のガイドライン：変遷と展望

Ⅰ．はじめに

　重症頭部外傷の治療と管理に関するガイドラインは，2000年前後，日米欧を中心に一斉に整備されてきた．端緒となったのは，米国の神経外傷財団（Brain Trauma Foundation：BTF）によるevidence-based guidelinesで，1995年に初版，2000年に第2版，2007年に第3版が公表され現在に至っている^1-3)．ヨーロッパでは，1997年に，ヨーロッパ脳損傷協会（European Brain Injury Consortium：EBIC）によりヨーロッパ全体のガイドラインとして発表された¹⁵⁾．このガイドラインは，医療事情の異なるヨーロッパ各国の実情に合わせて，専門家のコンセンサスに基づいて作成された．日本のガイドラインは，日本脳神経外傷学会の重症頭部外傷治療・管理のガイドライン作成委員会によって作成され，欧米とはまったく異なる医療保険制度や施設の多様性などを考慮して，専門家のコンセンサスに基づいたガイドラインとして，2000年に初版，2006年に第2版が発表された^11,12)．第3版もすでに印刷中の段階で，近日中に発表される予定である¹³⁾．筆者は，2003年に始まった第2版のガイドライン作成委員であり，2008年よりはガイドライン作成副委員長として，ガイドラインの改訂作業に関わってきた．

　本稿では，わが国の重症頭部外傷の治療と管理に関するガイドラインについて，第3版までの変遷と今後の展望について概説する．
