Ⅰ．はじめに

　硬膜動静脈瘻（dural arteriovenous fistula：DAVF）は硬膜上のさまざまな部位に発生し，病変部の構造は症例ごとに異なっている．術前の血管造影の理解と病態の把握が重要であり，いかにしてシャント部を閉塞するか，また到達できない場合の次善の方法は何かなど，治療のデザインを立てるためには十分な知識，経験，判断力が要求される．治療が奏功した場合の満足度は患者のみならず術者にとっても大きいものであり，それゆえに本疾患に思い入れが強い脳血管内治療医は多い．本稿では頭部に発生するDAVFの分類および塞栓術の具体的な手技，さらにdirectなhigh flow AVFの治療について述べる．
