（7）頚動脈ステント留置術の基本

本邦では，2008年4月に頚動脈用ステントとして初めて頚動脈用プリサイスと，プロテクションディバイスとしてアンジオガード（ジョンソンエンドジョンソン株式会社）が保険収載された．2010年8月には頚動脈用ウォールステントモノレールとフィルターワイヤーEZ（ボストンサイエンティフィックジャパン株式会社）が追加され，その後，中心循環系塞栓捕捉用カテーテルとしてガードワイヤ・プロテクションシステム（日本メドトロニック株式会社）も正式に保険収載された．また，2012年7月にPROTÉGÉ頚動脈ステントセットとスパイダー・プロテクション・デバイス（コヴィディエンジャパン株式会社），2013年1月には，外頚動脈と総頚動脈を同時に遮断するプロキシマールプロテクションディバイスMOMAウルトラ（メドトロニック）が保険収載され，選択肢はさらに拡大した．上記3種類のステントと4種類のプロテクションディバイス，各社から発売されているバルーン付きガイディングカテーテルの中から最適と考える組み合わせを選択して治療を行っている．これらを用いた頚動脈ステント留置術（carotid artery stenting：CAS）によって，安全かつ良好な結果を得るためには，ディバイスの構造や特徴，適応などを熟知している必要がある．
