（7）髄膜腫手術・2―convexity, parasagittal, falxの髄膜腫

Ⅰ．はじめに

　今回のレビューの対象とした，convexity, parasagittal, falxの各髄膜腫は，髄膜腫全体のうち，それぞれ25.6，11.6，11.3%を占め，これらで約半数を占めるという好発部位である³¹⁾．

　髄膜腫は，原発性脳腫瘍のうち，近年最も頻度が高い腫瘍であり，50歳代以降によくみられる．年間発生率は6人/10万人程度と見積もられ，女性の発生率は男性の約2倍である¹⁶⁾．また，剖検例の報告では，人口の2～3%程度に髄膜腫が発生すると考えられる²²⁾．

　上記疫学的報告より，①画像診断の普及と高齢化により発見頻度が増加したこと，②今後も増加するであろうこと，そのうち，③高齢者の無症候性髄膜腫の頻度がさらに増加するであろうこと，などが容易に予想される．

　今回のレビューでは，日常遭遇する可能性が最も高いこれらの髄膜腫の手術治療に関して，合併症の発生頻度を整理する．また前述した高齢者の無症候性髄膜腫が増加するであろうことより，経過観察や定位放射線療法などのデータにも触れ，若干の考察を加えて報告する．
