Azygos anterior cerebral artery aneurysmを合併したNoonan症候群の1例

Ⅰ．はじめに

　Noonan症候群は特異的な顔貌と複数の先天異常を有する症候群であり，出生1,000人から2,500人に1人の罹患率といわれている¹⁶⁾．遺伝形式として常染色体優性遺伝の形式をとり，責任遺伝子として12番染色体長腕のPTPN11が同定されている¹⁵⁾，Noonan症候群と診断された約半数の症例で同遺伝子変異が同定され，先天性心疾患，成長障害，両眼開離や耳介低位などの特異的顔貌，骨格異常，精神運動発達遅滞，血液凝固障害，リンパ管形成障害，停留精巣などの合併が知られている．Noonan症候群における予後に関与する因子としては特に先天性心疾患が挙げられ，そのほかにリンパ管形成異常，血液学的疾患なども挙げられている．脳血管障害による死亡例の報告は極めて少なく，脳動脈瘤を認めた報告も稀であり，われわれが渉猟し得た症例は4例であった^5,6,11)．またNoonan症候群におけるazygos anterior cerebral artery（azygos ACA）aneurysmの報告はなく，今回azygos ACA aneurysmを合併したNoonan症候群の1例を経験したので文献的考察を加え報告する．
