閉塞した橈骨動脈バイパスグラフトに間接的な側副血行の発達を認め治療完遂に至った海綿静脈洞部巨大内頚動脈瘤の1例

Ⅰ．経験症例

1．症例

　症例は60歳代の女性で，右眼瞼下垂，右眼球運動障害で発症し，前医のMRIで右海綿静脈洞部巨大内頚動脈瘤を指摘された．その時点では患者がそれ以上の精査加療を希望せず経過観察となったが，半年後に右顔面のしびれを伴うようになり当科に紹介となった．神経学的には右側の動眼神経，外転神経の麻痺および右三叉神経第1，2枝領域の感覚異常を認めた．右内頚動脈造影にて上記の動脈瘤を確認した（Fig.1）．右総頚動脈圧迫下での左総頚動脈造影，椎骨動脈造影では右内頚動脈領域への側副血行を認めなかった．右内頚動脈のバルーン閉塞テスト（balloon occlusion test:BOT）を施行したところ，数分で意識障害，左片麻痺が出現したため予定していた閉塞中のSPECT検査を行うことなく遮断を解除した．当時の治療方針に従って，まずは橈骨動脈グラフトを用いたhigh flow bypassを行い，2日後にコイルによるinternal trappingを行う二期的治療を計画した¹⁰⁾．
