傍鞍部腫瘍の自然歴と治療適応の検討について

Ⅰ．はじめに

　傍鞍部腫瘍性病変には，髄膜腫・下垂体腺腫・ラトケ囊胞・頭蓋咽頭腫・胚細胞腫・脊索腫・転移性脳腫瘍などがあり，こうした腫瘍の自然歴は，良性腫瘍である前者3つの腫瘍性疾患で特に重要である．近年では偶然こうした腫瘍が発見される機会が増えており，疾患を治療すべきか経過観察すべきかの判断には自然歴の深い理解が必須である．本稿では，①髄膜腫，②下垂体腺腫，③ラトケ囊胞の3つの傍鞍部良性腫瘍に関する最近の知見をまとめ，現在の問題点に関して議論したい．
