（10）手根管症候群

手根管症候群とは，手根骨と横手根靱帯からなるトンネル内（手根管内）の圧が上昇し，正中神経が横手根靱帯に押しつけられて絞扼を受ける病態である．半数以上は特発性といわれ，先天性に手根管が狭小している状態に，手関節の屈曲・伸展などによる物理的負荷が加わることにより発症すると考えられている．その他の発症の危険因子として，屈筋腱の腱鞘炎，関節リウマチによる滑膜炎，人工透析患者のアミロイド沈着，腫瘍，ガングリオン，骨折など手根管の内腔を狭める局所因子や，遺伝性圧脆弱性ニューロパチー，糖尿病性多発ニューロパチーなどの神経の脆弱性，妊娠，浮腫，甲状腺疾患，原発性アミロイドーシスなどの全身性因子などがあるとされる17)．また，女性に多く，閉経（早期のもの），妊娠および出産回数に関係するとの報告16)もあり，女性ホルモンとの関連も推定されている．さらに，重要な因子として，加齢と肥満がある．これまでの報告で，加齢とともに発症率が上がること，body mass index（BMI）が30を超え24)，腹囲が88cmを超える場合にも頻度が高いとの報告はよくみられる11)．
